AnnotationThe purpose of the National Confidential Enquiry into Perioperative Deaths (NCEPOD) is to identify remediable factors in anaesthesia and surgery, such as the provision of better facilities or different skills, and to make recommendations based on the findings. This report concentrates on the extremes of age: the under 16s, and those aged 90 years and over. The greatest problem seen in the older age group concerns the management of fluid balance. The report also highlights the need for a full diagnosis to be made before surgical intervention. Multidisciplinary reviews to develop good local working practices are required. NCEPOD expresses concern at the low postmortem rates in both age groups, and the need to improve hospital record systems as part of clinical governance duties. (RH).
