Annotation | According to the Department of Health (DoH), all health care professionals should be involved in measuring and improving their practice through audit. This article shows how the clinical audit process was used to measure actual discharge practice, generating results that were used to produce an action plan aimed at improving discharge planning. Although important in terms of evaluating practice, work such as this adds to the body of knowledge and builds an evidence base, which contributes to the debate about, and practice of, clinical governance.
