AnnotationA population of 220 depressed residents aged 65+ without severe cognitive impairment and living in self care units and hostels in Sydney, Australia participated in this randomised controlled trial of a shared care intervention. The intervention included: multidisciplinary consultation and collaboration; training of general practitioners (GPs) and carers in detection and management of depression; and depression related health education and activity programmes for residents. A control group received routine care. The Geriatric Depression Scale (GDS) and intention to treat analysis were used. There was significantly more movement to "less depressed" levels of depression at follow-up in the intervention than the control group. The outcome of depression in older people in residential care can be improved by multidisciplinary collaboration, by enhancing the clinical skills of GPs and care staff, and by providing depression related health education and activity programmes for residents. This article is followed by a commentary, "Beyond the boundary for a randomised controlled trial?" by Jonathan J Deeks and Edmund Juszczak. (RH).
