Annotation | Integration of case management proved to be a key variable in a demonstration at nine US sites of alternative models designed to enhance primary care of frail older people. Semi-structured interviews of participant case managers revealed a number of areas important to achieving successful integration: making favourable first impressions; building relationships; having proximity and contact; communicating; and demonstrating benefits to patients and doctors.
