Geriatric syndromes as outcome measures of hospital care
 — can administrative data be used?

AnnotationThis study uses two US Veterans Affairs (VA) databases - the Patient Assessment File (PAF), and the Patient Treatment File (PTF) - to determine how often they record the occurrence of two common geriatric syndromes: pressure sores and incontinence. There were 17,004 admissions to nursing homes from acute care in 1996. 12.7% had a pressure sore and 43.4% were incontinent. Of those with a pressure sore, the hospital discharge diagnosis listed 30,8% of cases, and incontinence was included correctly as a discharge diagnosis in only 3.4% of cases. Patients with more discharge diagnoses were more likely to have both conditions correctly recorded. From 1992 to 1996, small but significant improvements were noted in the correct recording of these geriatric syndromes as discharge diagnoses. Although administrative databases have proven to be a valuable tool for assessing and improving the quality of medical care, their accuracy in recording discharge diagnoses requires further improvement.
