Annotation | The importance of rehabilitation in promoting independence is recognised in current policy directives on the provision of care to older and disabled people, but has not been widely established within the spectrum of services provided by local authorities. The development of the Re-ablement Service by Worcestershire Community NHS Trust and Worcestershire Social Services Department (SSD) arose as the results of a combination of budget deficits, poor co-ordination between hospital and community services in the county, and dependence on residential and nursing home provision. The Re-ablement Service works by providing a comprehensive multidisciplinary therapeutic assessment and time-limited programmes of rehabilitation in the client's own home. It is aimed at reducing dependency and promoting service users' choice and control over their own care programmes. (RH). |
