AnnotationGlobal measures, such as the Barthel index, are often not relevant for nursing home patients being assessed in geriatric consultation. Goal attainment scaling is an individualised measure which has been used in community mental health and geriatric inpatient units. This Canadian study reports its use in nursing home patients. In 53 consultations in long-term care, an average of 1.7 goals per patient were set, covering medical, behavioural and ethical issues. Mean goal attainment scale at follow-up was 46 ±7, demonstrating that most of the goals were met. Goal attainment scaling is more responsive to change than any of the other measures examined. (RH).
