Annotation | Paranoid disorder is a morbid distortion of beliefs of a persecutory nature which sometimes reaches delusional intensity. Paranoid disorders in old age present special problems, causing distress to patients and family carers. They offer complex challenges to healthcare professionals, as they often indicate an untreatable underlying condition. This article outlines the various forms in which paranoia presents itself, and the author advises on its assessment and management. Although some causes of paranoid states are simple and treatable, the overall picture is less optimistic. Consequently, the disorder should be regarded as being a permanent condition with the potential risk of recurrence or further problems arising at any time throughout the remainder of the patient's life.
