Annotation | The Social Services Inspectorate (SSI) has described assessment and care management as the "cornerstones of community care". This paper focuses on the process of sensitively collating adequate information in the assessment of need in relation to the provision of community care services. A distinction is drawn between core and additional information and the use of a consumer or carer controlled notebook is explored with reference to creating a "full picture" of the consumer and his or her needs and concerns. The implications of using such a booklet is explored with reference to the "exchange model" of assessment, which aims to ensure that the consumer's voice is heard. (RH). |
