Annotation | In 1996, the need for more effective use of hospital beds for older people was identified in Kingston, Surrey. In response to delayed hospital discharges and bed shortages, local health and social services jointly initiated and funded a two-year pilot project to enable early discharge and to prevent unnecessary admissions. The shared care team (SCT) was developed to provide tailored care packages for patients from the Molesey or Esher areas who no longer required 24-hour hospital care. The author explains how the scheme works, and the benefits of pooling expert knowledge to provide a flexible service. (RH). |
