AnnotationThe Shrewsbury project represents an attempt to deal with some of the challenges of co-ordinating services, including how professionals make decisions. This article is a response to, rather than a commentary on, the issues raised in the preceding article by Philip Shelton. It comprises an analysis of how the best value framework can be applied to care processes for older people in primary and community care. Rather than propose solutions, it offers some ways into the problem, and suggests that more radical change may be required to achieve outcomes which are genuinely focused on users. (RH).
