Annotation | With high demands on health and social services, the impetus for the project described has its origins in EPICS (the Elderly Persons Integrated Care Scheme), which highlighted the need for closer collaboration between health and social services. The project, which was financed jointly for 10 months by Shropshire Social Services and Shropshire Health Authority, aimed to explore ways of developing services to maintain over-75s in the community, while working to improve multi-agency care provision. This involved general practitioners (GPs) - both fundholding and non-fundholding - community nurses and other NHS trust personnel, social services, and voluntary agencies all working together. This article outlines the project's aims and design, the findings of focus groups and a workshop, and future developments. (RH). |
