Annotation | A Medication Pass Supplement Programme set up in response to concerns over patients in a transitional care unit experiencing unacceptable weight loss during their stay is described, including two case studies. The programme targets specific residents possessing established nutritional risk factors. It involves nursing staff administration of a dietitian-reviewed and approved nutritional supplement with each oral medication pass for residents meeting the criteria for programme participation. Eleven months of data collected revealed that the programme results were encouraging. (AKM). |
