AnnotationThe Royal College of Psychiatrists in the United Kingdom has adopted the stigma of mental illness as its new campaign. As an integral part of this, the Royal College will have to consider the public perception that ECT (electroconvulsive therapy) is an archaic and barbaric treatment - a view being promulgated on numerous Internet Websites. This editorial reaffirms the profession's confidence in ECT as a treatment. The enigma refers to ECT as being one of the few treatments that are demonstrably so effective that have been so consistently vilified. ECT must be seen as part of a pragmatic approach to the treatment of severe depressive illness: further research is needed not into its efficacy, but into its acceptability. (RH).
