Non-medical predictors of quality of care of hypertension in elderly patients

AnnotationHypertension is a multiple-risk chronic condition which is highly prevalent among the older population. This Israeli study was designed to develop instruments for examining the quality of routine care of hypertension in older people, and, using non-medical predictors of quality such as doctor-patient interaction variables, to explain the variance of quality of care. The study population comprised 352 older people - 92% of one Jerusalem neighbourhood - who were members of Israel's largest sick fund. Multivariate analysis was performed using data from information sources including interviews, screening, observation, and examination of records. The findings indicated a plethora of deficiencies in the quality of routine care, mostly in the quality of screening and the control of hypertension. It was found that the outcome of care is primarily the result of the physician-patient interaction, rather than a lack of patient compliance. The author proposes a national programme using the instrument developed. (RH).
