Annotation | Data that could potentially explain early and late readmissions were collected over 2 months for all admissions to four Italian acute geriatric care units. After discharge, data on hospital readmissions were collected for 6 months. Readmissions were classified as "early" (within the first three months), "late", and multiple (2 or more readmissions irrespective of period). Those not readmitted (alive at home) were considered as controls. Those readmitted "early" were sicker than controls, had more social problems or behavioural symptoms, and were more functionally impaired. Early readmissions were also associated with unsatisfactory social conditions, living alone, severity of diseases, and cognitive impairment. "Late" readmissions had a significantly higher total score on the Cumulative Illness Rating Scale (CIRS), and were associated with comorbidity only. Those with multiple readmissions had more severe diseases, and more social problems. These findings suggest that interventions improving unfavourable social conditions may reduce the rate of rehospitalisation in geriatric patients. (RH). |
