Annotation | A study of over 75s living at home in the US determined to ascertain major findings in all domains of comprehensive geriatric assessment (CGA) - medical, functional, mental health, and social/environmental; the clinical value of repeating CGA annually; and factors affecting patient adherence to recommendations from CGA. The 202 subjects (mean age 80.8 years, 70% female, 95% white, 64% living alone) received annual in-home CGA and quarterly visits by gerontological nurse practitioners for 3 years. Detailed data was collected on clinical problems and subjects' adherence with recommendations. A continued yield of problems - most commonly medical - was identified, and recommendations made. 76.7% of subjects had at least one major problem identified that was previously unknown or suboptimally treated. One third had additional major problems identified during the second and third years. A constant number of therapeutic and preventive recommendations were made each year (11.5 per subject annually); adherence to treatment varied by type of recommendation. The research supports the practice of CGA, though further work is needed to determine factors that affect adherence to recommendations and important clinical outcomes. (RH). |
