Community care: enhancing or reducing choice?

AnnotationFindings suggest that in exercising their choice to return home from hospital, some older people, especially the more disabled and frail, have a poorer quality of life, and many may fail to reach their full rehabilitation potential. This article describes certain aspects of a small research project exploring the changes in the role of the physiotherapy service in rehabilitating older people in one North Wales county, following implementation of the NHS and Community Care Act 1990. The study was motivated by the observations of physiotherapists working in a local rehabilitation hospital for people over 75, that more of the older people were apparently being rehabilitated and discharged home. Far from being the first step in their return to a normal lifestyle, for some patients, discharge from hospital may represent the beginning of a lifestyle characterised by passive dependency and social isolation. The right to choose to be cared for in the community is a valuable one, but in the light of quality of life issues raised by studies such as this, it may not be the answer for all older people. (RH).
