AnnotationFalls and funny turns are very common in older people, and are the cause of substantial morbidity and mortality. In recent years, there have been important advances in our understanding of the causes of falls and how best to manage these in vulnerable older people. Given the magnitude of the problem, it is important that such research finds its way into clinical practice. Slowing of central integration is the most likely explanation for the increase in postural sway in older people, but disease, disability and drugs are more important in causing falls. Risk reduction and balance training can prevent falls, and further work is needed to find effective ways of dealing with this problem, as this article outlines. (RH).
