Clock drawing test in very mild Alzheimer's disease

AnnotationThe clock drawing test is a simple clinical assessment of multiple areas of cognitive function, and is often used as a dementia screening tool. The aims of this study were to determine the efficiency of the test to predict the presence of very mild Alzheimer's disease (AD), and to identify the elements of clock drawing that were most useful in differentiating cognitively intact older adults from those with mild Alzheimer's disease. Clock drawings from 41 outpatient cases of mild AD and 39 age- and education-matched older adults were scored using the Clock Drawing Interpretation Scale (CDIS) and another scoring system. Results showed that two or more errors in the depiction of the clock hands had a positive predictive value for AD. Normal hand placement on the clock drawing test did not exclude AD. However, when prevalence rates of dementia in community-dwelling older people are considered, these results suggest than normal clock hand placement indicates that dementia is unlikely. (AKM).
