Use of antipsychotic drugs in nursing homes

AnnotationThe degree of compliance with Omnibus Budget Reconciliation Act (OBRA) regulations and US Health Care Financing Administration (HCFA) interpretative guidelines regarding neuroleptic prescribing practices in nursing homes is measured. This data can be used to set threshold levels that would signal more intensive reviews. A second objective was to examine the compliance with each separate guideline, and to determine if `patterns' of compliance difficulty exist. 1573 nursing home residents' pharmacy records from eight nursing homes were reviewed August 1994-March 1996. 279 residents (17.7%) were actively taking antipsychotic medications. Mean compliance greater than 70% was found for: appropriate diagnostic indication, dosage within recommended limits, and documented appropriate target symptoms. Dosages were more likely to exceed limits in those patients with histories of major mental illness, particularly schizophrenia. Nursing homes were better able to comply with those guidelines that are most specific. Educational interventions now need to focus on behavioural interventions, monitoring of adverse effects, and efficacy. OBRA continues to have a significant impact on nursing home prescribing practices. (RH).
