Annotation | There is evidence that access to hospital based community care services (e.g. arrangements for home help or day care following discharge from hospital) promote better recovery and reduce the chances of readmission. This article analyses an attempt to challenge inadequate provision of information about services and restricted access to them. It presents the advantages of such an approach for users, and provides evidence of some of the major constraints on such initiatives becoming routine. It draws on a small-scale action research project carried out by a community care team and the author, in a rehabilitation hospital for older patients (aged 75 and over). The significance of the project for both hospital based and wider practice is discussed. (RH). |
