Prescription drug dependence in the elderly: the clinical pathway to recovery

AnnotationPrescription drug use, abuse and dependence in older persons present a difficult challenge for clinicians, pharmacists and others involved in the care of this population. Existing diagnostic models fall short in identifying the problem amid the co-morbidities of older age. Additionally, there is a lack of tested models for treatment of the problem. This paper attempts to build a rational and conceptual model for treatment of prescription drug dependence in older people based upon clinical pathways and case management concepts. Three case histories are reported which illustrate the importance of clinical pathway events, things that often happen during the phase of recovery commonly referred to as aftercare. The events may adversely affect the recovery unless they are well managed by the attending physician or other clinicians involved in the case. Identification of potential pathway events during formal programmatic phase of addition treatment is helpful in planning strategies to deal with these events as they occur. (AKM).
