Treatment of older adult problem drinkers: lessons learned from " the class of '45"

AnnotationIn this report the authors review their experience in guiding a large, age-specific outpatient treatment programme for older problem drinkers that has been in continuous operation for more than 15 years at the Portland, Oregon, Veterans Affairs Medical Center. Introductory sections discuss the special treatment needs, clinical heterogeneity, and goals of treatment for older problem drinkers. Next, the evolution of the programme is traced, from its beginnings as a simple "track" offering supportive social group treatment to a few otherwise healthy older veterans, to its present configuration as a diversified programme, with interdisciplinary staff and multiple strategies of case finding, assessment, treatment, and aftercare. Each element of the programme is described in detail. Treatment compliance and outcome data are then presented. Specifics of this particular programme are augmented by discussion of relevant reports from other programmes. Finally, several principles of treatment derived from this programme are proposed as recommendations for other treatment settings. (AKM).
