AnnotationThis inspection of discharge arrangements of eight local authorities follows up inspections reported by the Social Services Inspectorate (SSI) in 1995, `Moving on' and `Moving on: a further year'. The report looks at social service departments' (SSDs) role in inter-disciplinary assessment, care planning, and rehabilitation. Findings are also presented regarding: referral; user and carer involvement and satisfaction; review; information for users, carers, staff and managers; strategic agreements; and settling disputes. Examples of good practice are given. Overall, discharge arrangements were found to be improving. However, co-ordination and communication between community-based health or social services staff remained limited. (RH).
