AnnotationThe Waltham Forest Hospital at Home scheme was set up to prevent hospital admission, facilitate early discharge, and offer packages of care tailored to individual patients. This final report discusses the developmental process; and presents information on operational issues concerning care and its outcomes. The authors recommended the continuation and development of the Hospital at Home service, pointing to improvements in liaison and collaboration between ward staff, specialists and general practitioners (GPs).
