Neuroleptic prescribing to the community elderly in Nottingham

AnnotationAs part of a larger project to examine practice throughout Nottingham, this pilot study set out to assess the pattern and monitoring of prescribing oral neuroleptics to older patients registered with general practitioners (GPs). The scale of use in residential and nursing homes, relative to other people living at home was also examined. Four general practices provided data regarding patients recorded as receiving repeat prescriptions for thioridazine, chlorpromazine, promazine, haloperidol or trifluoperazine. Older patients were found to be more likely to be consumers of neuroleptic medication than their younger counterparts. If the results obtained were extrapolated nationwide, then approximately half of those receiving repeat prescriptions for the commonest oral neuroleptics are older people, of whom about half are in nursing or residential care. Patients in nursing/residential homes suffering from dementia formed the largest group of recipients, but have the least monitoring by psychiatric teams. The result highlights a need for close partnership between primary care, community care, and psychiatric teams to ensure adequate monitoring and implementing of psychological strategies to minimise their use.
