AnnotationThe two papers discussed, both by Bernard Tomlinson, Gary Blessed and Martin Roth, appeared in the Journal of Neurological Sciences in 1970 (vol 11, pp 205-242) and 1968 (vol 7, pp 331-336) respectively. They are regarded as a milestone in the psychiatry of old age, and in the link between pathological brain changes and clinical features of dementia during life. The introduction summarises the main points of the two articles. An author of the original studies outlines the circumstances of the research. Despite notes of caution, the authors' findings were quoted extensively as representing actual percentages of causes of dementia, ignoring their descriptions of `probably' or `possibly' Alzheimer's disease or multi-infarct origins. Some cases of cortical Lewy body disease were almost certainly included, which the authors failed to recognise. The commentary points out that not all the concepts originally advanced have stood the test of time. It must be remembered that this work was done without the benefit of computer assisted analysis, and relied on laborious manual methodologies. (RH).
