Discharging older people from hospital to care homes - implications for nursing care

AnnotationDischarge of older people to care homes has not been widely researched, yet it involves a major life change for older people. This study aimed to examine the experiences of older people and those close to them to discharge from hospital to care homes, and to explore the experiences and views of medical and social care staff involved in the process, focusing on the role of nursing staff. Findings revealed that many older people appeared passive and did not ask health care or social care staff for advice on aspects of moving to a care home. Although there was general agreement that older people should have access to information and support, the main professional groups involved all felt unable to do this, and assumed someone else would. The authors suggest strategies of improving nursing knowledge and clarifying the role of nurses in supporting older patients moving into residential or nursing home care. (AKM).
