Annotation | This article is an edited version of the author's winning entry in a competition on assessment run by the RCN (Royal College of Nursing) Forum for Mental Health and Older People. Madeline Armstrong describes the importance of forming a therapeutic interpersonal relationship with the patient and the carer at the assessment stage, as this underpins future care. Whilst using this approach for assessment can be very time-consuming, the benefit is that it is comprehensive, and once all the facts have been recorded, they are available for future reference.
