AnnotationGeriatric drug therapy interventions involve the encouragement of appropriate therapy and prevention of problems with medications. Exponentially rising health care costs of older people have brought radical suggestions for rationing of health care. Drug-related problems of non-compliance and adverse drug reaction and interactions may influence the need for up to one third of hospital admissions and half of nursing home admissions of older people in the US. The author reviews types of drug-related problems and associated factors including under-recognition and misdiagnosis of problems. The pharmaceutical industry has come under scrutiny by the US Food and Drug Administration (FDA), which has conducted a review of post-approval risks of drugs released from 1976-85: of 198 drugs approved, 51.5% had serious post-approval risks, with many problems not evident until after 5 or more years on the market. The article concludes by discussing of the role of pharmacists in geriatric disease management. This article also appears as a chapter in `Geriatric drug therapy interventions', edited by the author (Haworth Press, 1997). (RH).
