Annotation | Community care for older people with dementia may be fragmented, and lack of interagency collaboration may mean that care packages fail to meet the needs of both dementia sufferers and their carers. The CARD Project (Coordination of Assessment and Resources in Dementia), established in 1995, is a joint initiative between primary care and social services in Glasgow. This article looks at how the project achieved effective inter-agency collaboration in providing community care to people with dementia. (AKM). |
