Post-acute services to older patients with heart disease

AnnotationPost-acute care is care of a patient who is no longer in need of acute care but is not fully recovered from the illness requiring hospitalisation. This US study investigated the assistance provided to 209 older patients with heart disease who received discharge planning before returning home from the hospital. Telephone interviews at two weeks post-discharge focused on their needs and the assistance received. The findings revealed that patients were in great need of assistance after discharge from hospital, and that families provided the majority of the care. Formal service use was most frequent in nursing care, bathing and grooming, but limited in areas of highest need, such as shopping, cleaning and transport. Of the patients interviewed, 30% reported insufficient help with personal care. Patients rated the quality of care provided by formal sources higher than care provided by informal sources. The authors question the ability of families to meet the demands of post-acute care. (AKM).
