Annotation | The authors present a brief review of where research on Alzheimer's disease (AD) is heading. Evidence confirms that the generation of Aß amyloid from the amyloid precursor protein is the central pathway in AD. Genetic and environmental causes and their risk factors are summarised. The lack of a small animal model of AD has been an impediment to development of rational treatments, but one may soon be available. The position on clinical trials of drugs awaiting approval or clinical evaluation is summarised. Some predictions are given on concepts on management of AD in 2008: genotype screening, analysis and counselling; presymptomatic diagnosis; rational preventive treatment - drug based or gene based; and changes in lifestyle, avoiding environmental risk factors.
