Annotation | Use of a single, comprehensive assessment tool for over-75s' annual health checks should provide professionals with a baseline from which to plan integrated care. This article describes a project developed by the health visitor for older people (HVOP) and the occupational therapist (OT) from Forest Healthcare Trust, on behalf of the Chingford and Woodford Fundholding Group. It outlines the assessment's content, and the role of project's team members. Brief results of a pilot patient satisfaction study at the end of the first year and an evaluation are presented. The benefits for a small team of pooling knowledge, skills and separate professional perspectives, and of raising awareness among other members of the primary healthcare team serve to highlight the project's usefulness.
