Annotation | This article summarises the types of care in the US for older adults with acute conditions: ACE (acute care for elders) units, home hospitalisation, and subacute care units. For high-risk older adults, health-care organisations are looking to contain their costs through use of: geriatric evaluation and management (GEM); case management (CM); interdisciplinary home care (IHC); pro-active primary care at `co-operative health care clinics' (CHCCs); and adult day health care (ADHC) such as provided by On Lok in San Francisco. Most older adults are low-risk; and preventive care, monitoring and treatment is likely to be less complex. (RH). |
