Annotation | Teamwork is seen as the solution to failures in inter-agency coordination of care. The aim of this report is to provide an overview of different approaches to multidisciplinary team working in the context of dementia. It describes various team models in place in the United Kingdom (UK), and identifies the features which have shaped organisational and professional development. The final chapter outlines a strategic framework for community dementia teams, which involves all stakeholders across health, social work services, housing, the voluntary/independent sectors and users and carers.
