Annotation | The underlying concept of the S/HMO (social health maintenance organization) in the US is the possibility and desirability of redirecting a portion of an HMO's resources expended in acute care of frail older people towards their long-term care (LTC). This article discusses the initial plans for the second generation S/HMOs, in the light of experiences of their S/HMO predecessors and the current policy and practice in which the new S/HMOSs will be launched. The authors focus on the theoretical and policy concerns; their comments are based on a detailed report for HFCA (Health Care Financing Administration) on the incentives that affected the first generation S/HMOs, and on their experience in providing technical support assistance. |
