| Annotation | Hospital discharge is a transfer to community care, the central stated aims of which are the enhancement of independence and promotion of user choice, to be achieved through the process of consultation and user participation. The research upon which this paper is based followed a number of older people through the processes of hospital discharge, focusing upon those who were assessed during a home visit prior to discharge by a hospital occupational therapist and for whom equipment and/or housing adaptations were recommended. This article discusses the older people's responses to such recommendations, and questions the extent to which the stated objectives of current community care policies are being met. It argues that hospital discharge procedures are led more by the exigencies of emptying beds than by the policy objectives of community care. The episodic nature of the assessment, coupled by patient anxiety about the purpose of the home visit and professional dominance of the process, militated against consultation, while safe discharge, rather than enhancing independence, drove professional practice. |
