AnnotationThe author, who is affiliated with Melabev Community Clubs for Impaired Elderly in Jerusalem, Israel, proposes the use of a standard form to be sent to nursing home by adult day care staff, when a client with dementia stops attending adult day care and moves into an institution. The form would contain non-confidential information that the adult day care staff have learned about their client that could be useful to the institutional staff in their efforts to treat the new client as a unique person. The use of such a form will be beneficial for the client as well as the staff members of both adult day care and the institution and will fill a gap in the continuum of care.
