AnnotationReactions from older people to the concept of the Health and Social Care Passport. Information was obtained through focus group interviews and participants also completed a draft version of the passport. The views expressed in the groups did not always match experience in completing the draft version. The term 'passport' is not appropriate for this type of document and a better name is required. Contents: Objectives and method; Background; Reaction to the Health and Social Care Passports; Survey of elderly peoples' views; Summary and conclusions; Appendices; sample pages from draft passport; extent of completion of draft passport. [RHb] [EPICS collection].
