Care management: is it working?

AnnotationCare management is the term used to describe both the separation of staff responsible for assessment and care planning from those responsible for service provision, and the process by which the former activities should be undertaken. The aim of this research project was to evaluate how effectively care management was being implemented in four local authorities (Durham, Berkshire, Lambeth and Westminster), with a view to identifying key factors which assisted or impeded effectiveness. In particular, the project examined whether any particular model of care management had decisive advantages over any other. The following features of care management were examined in each local authority: the clients; client-centred care; care co-ordination; and the role of the care manager. The report concludes with a number of recommendations.
