Annotation | In the US, homecare policy has shifted from care in the home provided by family and doctors making housecalls, to hospital, GP surgeries and nursing home back to home once more. At the same time, families have been increasingly become geographically dispersed. This paper outlines who, where, and what are the health and social service needs of frail older people living in the community. Some examples are outlined of services provided by partnerships supporting older people living at home. (RH). |
