Annotation | Social workers and health professionals in Bromsgrove, Worcestershire, are testing a new approach to the problem of working as a multi-disciplinary team, to provide intensive home support to patients who do not need acute medical help, but do need low-tech intervention from health and social services. The multi-disciplinary team includes: practice-based social workers; social services-based home carers specially trained in rehabilitation; general practitioners (GPs); community trust occupational therapists and physiotherapists; nurses who specialise in caring for older people; and day hospital staff.
