Annotation | Many so-called `bed-blockers' no longer need acute care, but are not ready to return home. The authors suggest that intermediate care - which focuses on helping people through the transition from hospital to home - may be the answer. The intermediate stage of health care has been defined as the range of services designed to facilitate the transition from medical dependence to functional independence: the objectives of care are not primarily medical, and a clinical outcome of recovery is desired. This article outlines the benefits for patients and service providers, and the challenges for health care professionals. (RH). |
