Annotation | The demonstration examined was designed to determine whether Medicare recipients living in five rural areas of Pennsylvania would use health promotion and/or disease prevention services if they were covered. It also aimed to determine whether local doctors and hospitals would be willing to provide such services if they were paid by Medicare. Two different approaches to covering these services were tested. In one model, hospital-based "centres" received a capitation fee, and were responsible for providing both the risk factor screenings and the newly covered services. The second model was designed to examine the impact of covering these services using the usual fee-for-service approach. In this evaluation, the authors discuss the design of the demonstration; examine the use of services under demonstration; and assess the impact of covering health promotion services on: the use and cost of traditional medical care services; and mortality. They compare their findings with those published to date from other demonstration projects. They also explore the limits of using the demonstration process to test the implications of covering services such as health promotion type services. (RH). |
