AnnotationThis report examines how the local authority, four hospital trusts, and primary care teams were fulfilling their obligations under the New River Health Authority and Enfield Social Services Department `Arrangements for assessment and discharge of people in hospital' (January 1995). It also examines the extent to which health agencies were meeting the requirements of the Department of Health (DoH) guidelines, `NHS responsibilities for meeting continuing health care needs' (February 1995). The report provides information on key themes: strengths and weaknesses of current arrangements; performance; roles and responsibilities; service deficits; training; and providers' experiences. The report makes recommendations to build on existing strengths and to deal with weaknesses. (RH).
