Annotation | This report presents the multi-agency Community Care Project Group with findings of an appraisal of the strengths and weaknesses of arrangements for hospital discharge in Haringey. It presents information on performance, roles and responsibilities, service deficits, training, providers' experiences, and users' and carers' experiences. It examines the wider context of continuing change within the NHS (National Health Service) and social care sector. The author concludes that substantial service development needs to be undertaken in a range of areas, and identifies a strategy for improving the quality of discharge arrangements. (RH). |
