Annotation | With the shift away from long-stay hospital beds, there is urgent need to ensure that patients returning home or to a nursing home can get high quality multidisciplinary health care. The author describes the work of a pilot study team in the Airedale NHS Trust area of West Yorkshire, which aimed to stop patients slipping through the net when discharged from hospital into community care. A greater understanding of each other's roles in the team helps forge effective links, and ensures better patient care. |
