AnnotationThe aim of this research was to investigate the effect of the implementation of the NHS and Community Care Act 1990 in April 1993 on the discharge of frail older patients from hospital, focusing on discharges to institutions and the outcome of their relocation to continuing nursing and residential care. Data from 1992 (before the Act was implemented) and 1994 have been used for making comparisons for the five elderly care units (ECUs) on patients' characteristics, patterns of discharge, average lengths of stay, readmissions, and institutionalisation rate (the percentage of discharges to institutions). The data suggest that in 1992, the five ECUs were practising medicine in diverse ways. The pressures from the market oriented NHS coupled with financial responsibilities resting with local authorities suggest that the length of stay in ECUs after discharge from hospital is likely to decrease.
